MyHealth & Long term conditions

Around 15 million people in England have one or more long-term conditions. The number of people with multiple long-term conditions is predicted to rise by a third over the next ten years. People with long-term conditions are the most frequent users of health care services, accounting for 50 per cent of all GP appointments in Hillingdon.

Our growing programme of workshops and local support connections is intended to provide those living with long-term health conditions the help and confidence they need to make healthier choices and lead fuller, more active and healthier lives – through being more informed and by playing more of an active role in their health and helping them to reduce the amount of trips to the GP or local urgent care centre.
